What do I need to join? <\/strong><\/p>\n\n\n\n

For each skater, RAJRD requests <\/p>\n\n\n\n

– a completed and SUBMITTED online registration form
– liability waiver
– a copy of valid health insurance card
– 1 health history form
– 1 medical consent form (authorization for emergency medical treatment)
-parent code of conduct
-skater code of conduct
-photo release form<\/p>\n\n\n\n

Paper copies of Liability Waivers, JRDA Health\/Medical and Photo release forms are provided. Forms may be completed and emailed in advance if it is more convenient for your family to do so.<\/p>\n\n\n\n

Skaters playing in sanctioned level 3 games may have additional requirements to complete such as JRDA skater registration<\/a> and an age waiver. <\/p>\n\n\n\n

All Skaters, their Parent(s)\/Guardians\/Family members and non-parent Roll Models are required to adhere to the Rochester Area Junior Roller Derby Code of Conduct that applies to their role. The Skater Code of Conduct is reviewed with skaters annually. Regardless of whether the firm has been signed, agreement to follow the code of conduct is implied simply by participating inRAJRD events and practices.<\/p>\n\n\n\n

The forms below are only required in certain circumstances. <\/p>\n\n\n\n

Appendix D- Medical Clearance Form Template is one option\/example of a form clearing a skater to return to play after injury or suspected concussion. Your doctor may have their own form they prefer, which is fine!<\/p>\n\n\n\n

JRDA Skater Registration. <\/a> Register skaters by creating a parent account here<\/a>, and then adding any skaters. If names or numbers change as skaters mature, be sure to keep coaches apprised as there are additional forms to make updates! <\/p>\n\n\n\n

JRDA Age Waiver. <\/a> Scroll down to find this form which is completed by the skater, parent AND a coach from our league. This form is required for skaters that turn 14 after September 1 of any given season. To be eligible for Level 3 play, skaters must have fully passed their level 3 skill assessments and rules tests. Skaters 13 and under (as of Sept. 1) cannot be added to the charter until this form is complete!<\/p>\n\n\n\n
